1. a) Brief overview of the Program Division b) Brief overview of the GC7 grant component in the Program Division:

a) National AIDS Control Organization is a division of the Ministry of Health and Family Welfare that provides leadership to HIV/AIDS control programme in India through HIV/AIDS Prevention and Control Societies. In 1986, following the detection of the first AIDS case in the country, the National AIDS Committee was constituted in the Ministry of Health and Family Welfare. In 1992 India’s first National AIDS Control Programme (1992-1999) was launched, and National AIDS Control Organization (NACO) was constituted to implement the programme.

National AIDS and STD Control Programme (NACP) Phase-V (2021-26) is a Central Sector Scheme fully funded by the Government of India with an outlay of Rs 15471.94 crore. The NACP Phase-V aims to take the national AIDS and STD response towards the attainment of United Nations’ Sustainable Development Goals 3.3 of ending the HIV/AIDS epidemic as a public health threat by 2030 through a comprehensive package of prevention, detection and treatment services.

b) The HIV GC 7 Grant was allocated for USD 155 m, of which USD 100.29 m was allocated to NACO and USD 54.71 m was allocated to four Non-Government Principal Recipients. With the changing international funding landscape, the grant allocations for GC 7 have been revised. While the Non-Government Principal Recipients’ grants are on input based financing, NACO’s grant is a Payment for Results grant wherein the disbursement is based on the achievement of certain ‘Disbursement Linked Indicators (DLIs).

India HIV/AIDS Alliance (IHAA), focuses on delivering integrated, community-centric care, support, and prevention services for People Living with HIV (PLHIV), key populations, and underserved groups. IHAA ensures effective implementation of the Vihaan program (CSC 2.0), supports interventions in prisons and other closed settings, expands outreach through virtual platforms under NetReach, and empowers communities via structured Community System Strengthening (CSS) efforts. Additionally, IHAA contributes to national priorities by supporting the Elimination of Vertical Transmission of HIV and Syphilis (EVTHS) and strengthening targeted case finding through Index Testing. Emphasizing differentiated service delivery, system linkages, and person-based approaches, IHAA operates across 11 states to reduce stigma, improve health outcomes, and reinforce the national HIV response.

SAATHII implements the interventions with community partners in ten states and union territories, including the five southern states and union territories and the northern states of Haryana and Punjab to strengthen the Care & Support Program by setting up 78 CSCs, facilitate CSS activities, implement the Prison & OCS Intervention and strengthen TI & LWS training through Kshamta Kendras.

HLFPPT through Project SSHAKTI (Strategizing and Strengthening HIV/AIDS & TB Initiative) bolsters the Care & Support Program by establishing 74 Care and Support Centres (CSCs) in 15 states and UTs in partnership with Sub-Sub Recipients (SSRs), facilitate Community System Strengthening (CSS) activities via three Sub Recipients (SRs), and implement Prison and Other Closed Setting (OCS) Intervention.

Plan India - As part of GC 6, One Stop Centres (OSC) were envisaged to offer a large spectrum of health and non-health related services to high risk populations especially Transgender persons, People Who Inject Drugs and Bridge Population (Truckers & Migrants) through a community based single window approach. As part of GC 6, 74 OSC were established. In GC 7 Plan India continued working with the 74 OSC, in the same geographies (approved by NACO, in concurrence with respective SACS and local community leaders)

Care and Support Centres: To revamp the existing CSC into CSC 2.0 with them aim of strengthening differentiated care and support services to PLHIV clients. The CSC 2.0 would focus on peer-led outreach to address key intervention areas as follows- ICTC to ART linkage, retention in care, adherence support for PLHIV on ART, and elimination of vertical transmission of HIV and syphilis.

Key Activities:

  • Conduct outreach and home visits to track and support PLHIV, especially LFU cases.
  • Provide peer counselling and psychosocial support, especially to key populations, children, adolescents, and pregnant women.
  • Ensure early linkage to ART for newly diagnosed PLHIV and follow-up for viral load testing.
  • Support elimination of vertical transmission (EVTHS) through follow-up of HIV-positive pregnant women and HIV-exposed infants.
  • Facilitate HIV testing for family members and partners through index testing.
  • Provide ART refill services in special settings and difficult-to-reach areas with SACS/NACO approval.
  • Link clients to social protection schemes and welfare entitlements.
  • Conduct Support Group Meetings (SGMs) for PLHIV to promote positive living and treatment adherence.
  • Undertake inter-district and interstate tracking to re-engage clients who have migrated.
  • Coordinate with ART Centres and stakeholders for referral and linkage to services such as OST, TI, TB, legal aid, and nutrition.

Community System Strengthening: The activity includes Constitution of District Community Resource Groups (CRGs); Rolling out of CLM and Strengthening of State Level KP networks.

Key Activities:

  • Constitute and support Community Resource Groups (CRGs) at district and state levels to address service barriers, review action taken reports, and advise SACS/NACO on redressal strategies.
  • Operationalize Community Led Monitoring (CLM) using community-developed tools to assess accessibility, availability, stigma-free environments, and client satisfaction at ART, ICTC, TI, OST, and STI/RTI facilities.
  • Train and deploy Community Champions from key populations and PLHIV groups to represent community concerns, mobilize peers, and advocate for improved service delivery.
  • Facilitate regular feedback loops through community scorecards and structured grievance redressal, feeding into CRG deliberations and national/state level response systems.
  • Build capacity of communities and CBOs/NGOs through structured training modules on advocacy, monitoring, rights literacy, and systems engagement.
  • Engage PRIs, health officials, and other stakeholders to create an enabling environment for stigma reduction and community ownership of HIV service delivery.

Prison & OCS Intervention: The aim is ensure HIV and TB prevention and treatment services for all inmates living in prisons and other closed settings. Prevention Education, Screening and treatment linkage for STI, TB, and Hepatitis C, Post-Release Linkages etc. services are included.

Key Activities:

  • Conduct peer-led awareness sessions focused on holistic health, with emphasis on HIV, TB, and STIs.
  • Develop customized IEC materials for different inmate categories (undertrial, short-term, long-term).
  • Promote harm reduction and safer behaviours through sustained SBCC efforts.
  • Sensitize prison and OCS officials to support health interventions.
  • Train medical and paramedical staff for effective delivery of services.
  • Establish and strengthen SA/F-ICTCs, LACs, S-OSTs, DDACs, and DOTS centres within prisons.
  • Ensure continuous supply of medicines, testing kits, and consumables.
  • Engage families of vulnerable inmates through counselling and testing support.
  • Promote regular and on-entry testing and linkage to treatment for all inmates.

 

Virtual intervention: The activity is designed to extend HIV prevention, testing, and linkage-to-care services to key populations who are active on virtual platforms. This digital outreach model enables real-time interaction with hard-to-reach and high-risk individuals—especially those not covered through physical outreach—via online spaces such as social media, dating apps, and chat platforms.

  • Identify and engage high-risk individuals on virtual platforms using trained Online Outreach Workers (OOWs).
  • Conduct virtual risk assessments and counselling through secure messaging and voice/video interfaces.
  • Provide online referrals and linkage to physical services such as HIV testing, STI screening, PrEP/PEP, ART, and mental health support.
  • Enable HIV testing by providing information on access points.
  • Maintain data confidentiality and privacy through encrypted communication and coded profiles.
  • Promote demand generation for HIV services through digital campaigns, influencer collaboration, and targeted content.
  • Track and support linkage to care and retention via virtual follow-ups and peer support.
  • Reach migrants, MSM, TG, and youth who may be less accessible through conventional outreach mechanisms.

The HLFPPT is implementing 74 CSCs across 12 states to support activities such as LFU tracking, Viral Load Testing, Index Testing, HEI testing, and other priority indicators to enhance service delivery for People Living with HIV (PLHIV).

  • 74 Care and Support Centres in 12 states ensure continuum care for PLHIV, focusing on adherence counselling, mental health, and quality of life improvements.
  • Collaboration with 165 ART centres, 3 Sub-Recipients, and 74 grassroots organizations to enhance service delivery.

Community System Strengthening: Under the CSS component, HLFPPT will ensure the training of Community Champions on the CLM module, facilitate DCRG meetings, and support the formation of State-Level Networks (SLNs).

Prison & OCS Intervention: The aim is ensure HIV and TB prevention and treatment services for all inmates living in prisons and other closed settings. Prevention Education, Screening and treatment linkage for STI, TB, and Hepatitis C, Post-Release Linkages etc. services are included. It also includes to strengthen strategic partnerships by working with state authorities to improve health outcomes in marginalized settings.

HLFPPT ensures the intervention in the 15 States which includes about 597 Prisons and 146 Other Close Setting.

Plan India

One Stop Centres

Activity is being implemented pan India.

The OSCs offer a large spectrum of health and non-health related services to high-risk populations especially Transgender persons, People Who Inject Drugs and Bridge Population (Truckers & Migrants) through a community based single window approach.

  1. Site Mapping: In their respective location, the OSCs have mapped out the hotspots and congregations where the Target Population is available. Mapping allows to plan for saturation of the typology with services and ensures there is no duplication with other service providers. Mapping is reviewed on a quarterly basis to add new hot spots and move out of older hotspots (if required)
  2. Demand Generation Awareness & Health Camps and CBS: At the OSC premises or at the hotspots, awareness generation is conducted to make the community and the typology aware about the services (both heath and non-health) available in the OSC. Making clients aware on their vulnerabilities towards HIV. HIV screening, Syphilis screening (subject to availability of screening kits), TB 4S and other NCD (hypertension and diabetes) screenings are also available at the DGA location. Along with these health services beneficiaries are also sensitized on various related social welfare schemes and based on their demand liked to these schemes.
  3. Networking Meeting: Alongside with the site mapping, stakeholder mapping is also done. Networking Meeting is a platform to engage meaningfully with the key program stakeholders.
  4. Outreach and Risk Assessment: At the time of registration of clients, a risk vulnerability assessment is done. Based on the RVA outreach is planned. The objective of the outreach is to reduce risks through continuous follow up. Each month a line list of due services for already registered client is generated and based on the same the outreach visits are planned to ensure the uptake of the services scheduled. During outreach it is also ensured that any uncovered population is meaningfully engaged with the program to ensure saturation of these key and bridge populations in the district.
  5. Commodity Distribution: As part of the harm reduction strategy and to motivate beneficiaries to lead a healthy life, typology-specific commodities such as condoms, lubricants, and needles/syringes are periodically made available to beneficiaries at the OSC. These commodities are sourced through respective State AIDS Control Societies (SACS), while additional support is mobilized through Corporate Social Responsibility (CSR) funds.
  6. Emergency Support Fund: The Emergency Support Fund plays a vital role in safeguarding the well-being of the most vulnerable and financially disadvantaged clients by enabling timely access to essential health services and treatment. These interventions are often critical to preserving life and ensuring a minimum quality of life, especially for clients belonging to marginalized key populations. In the absence of such support mechanisms, individuals facing acute medical or psychosocial challenges may find themselves in life-threatening situations, unable to navigate or afford mainstream care pathways. A detailed Standard Operating Procedure (SoP) has been developed to guide the utilization of this fund, identifying nine specific conditions or emergency scenarios where this support may be extended, each with clearly defined funding limits to ensure equitable and transparent use. The fund covers transportation, medicine, preparation of affidavits and clinical investigations etc.
